- Description of Research Activity and Interests: 

My research interests and activity involve the following:

Α) Descriptive anatomy, clinical and radiological anatomy, current medical imaging.

- In an original research study, we evaluated 73 cadavers for anatomic variations of the right hepatic duct, as well as their respective frequencies in the Greek population. The thorough knowledge of the hepatic duct anatomy is very important in the saferty of surgical procedures that involve the liver and the biliary tract. From this study we conclude that almost 35% of the Greek population carries at least one anatomic variation of the right hepatic duct. The importance of the knowledge of the anatomy in the area is highlighted, as well as the implications on the surgical practice of liver transplant and biliary tract operations.

- In an original study, we evaluated the methods for precise anatomical mapping of the anorectum in patients with complex and complicated fistula – in – ano. The precision in the anatomic description of the tissues involved is crucial and is considered the most effective precaution in the surgical intervention that follows. The anatomical mapping was completed using imaging studies (endorectal US and MRI) as well as surgical dissection of the anorectum.

- In another study we present a rare case of post-traumatic implantation of splenic tissue in the subcutaneous tissue. Splenosis, as it is most commonly known, is a common benign disease that is most frequently diagnosed in trauma patients and involves abdominal viscerae, with the mechanism of occurrence being the rupture of the spleen and the self – transplant of functional splenic tissue. It is most frequently discovered within the peritoneal cavity. In our study, we correlate the architectural structure of the ectopic splenic tissue with the histologic structure of the spleen and the anatomical area of the finding. We also analyze the existing studies on splenosis, as well as the diagnostic and therapeutic approach of such cases.

- We present another case of infiltration of solitary internal jugular vein from thyroid cancer, a rare complication of this disease, and describe in detail the anatomical basis of the surgical technique for the reconstruction of the defect of the IJV, after excision, with self – transplant of saphenous vein.

- In a clinical case study, we present a patient with primary hyperparathyroidism, in whom the diagnostic evaluation with US was challenging, due to rare anatomical structure of the adenoma. The function of the adenoma was correlated with other imaging studies (MRI, Tc99m Scan) for proper diagnosis. We also conducted a review of the medical literature on this rare anatomical presentation of the parathyroid adenoma.

- In an original study, we investigated the alteration of the structure of the human body in teens, correlating the BMI with personal, social and family characteristics of the population that was studies. The results highlited the importance of teen obesity in Greece, while the age and lifestyle of the parents were positive prognostic factors for increased BMI in teenagers.

- In another original study, we report the results of a diet intervention in juveniles of school age. After the intervention, we measured the improvement of BMI as well as the calorie intake and their quantity (fat intake) in the intervention group, highlighting the benefit of such actions in the prevention of obesity.

- In a study we present a rare anatomical and histopathological alteration in the ureter of a female patient with endometriosis. Endometriosis of the urinary tract is rare, while sole endometriotic nodules in the ureters are even rarer. In this case of endogenous endometriosis of the ureter, we also review the existing literature, and report the importance of the specific histologic characteristics in its diagnosis.

 

Β) Research studies in functional anatomy, descriptive anatomy, anatomic variations of organs and tissues, with emphasis on surgical anatomy and the implications in current surgical technique.

- In an original paper, we investigated the disruption of the anatomical structure of the esophagic foramen in patients with giant esophageal hernias, as well as the surgical reconstruction of the defect with prosthetic mesh. We reported the results of this technique in 45 patients and analyzed the efficacy and safety of the laparoscopic approach.

- In venous catheterization, it is possible to mistakenly insert the catheter in an artery instead of a vein. We report a case of mistaken placement of a venous catheter in an arterial branch, and analyze the steps required in order to identify and manage such cases, contributing to the safety of venous catheterization.

- The presence of an inflamed appendix within a femoral hernia is a rare condition, with serious complications that often is misdiagnosed. We extensively studied a case of De Garengeot’s hernia, describing the steps of a successful preoperative diagnosis. We also described the imaging and descriptive anatomical findings with the surgical anatomy of the defect, while reviewing the total of the existing literature on this rare clinical entity. The importance of the special anatomic characteristics of the area were highlighted, in relation to the diagnostic steps and the therapeutic approach.

- We reported the first placement of peripherally inserted central catheter (PICC) under ECG control in Greece. There was a specific emphasis on the descriptive anatomy of the superficial and deep venous plexus of the upper limb, as well as the technical and anatomic details of the technique, under US guidance. The correlation between vascular anatomy and imaging is analyzed for the safety of PICC insertion. Also, we discuss the advantages and drawbacks of PICCs, concluding that insertion of PICCs has significant advantages in terms of safety and cost versys central venous catheterization.

- In a case presentation study, we report a case of male lobular breast cancer, a very rare entity, given the absence of lobules in the male breast. We also conducted a through literature research, and highlight the importance of genetic counseling and endocrine monitoring in such cases.

- In another case report, we describe a very rare case of mucinous tubular and spindle cell (MTSC) renal carcinoma, in a 67-year-old patient. An extensive review of the existing literature was conducted, that highlighted the importance of precise differential diagnosis of MTSC, with regard to the specialized therapeutic approach and the specific structural characteristics of the tumor.

- The role of laparoscopic surgery in the management of GERD, morbid obesity and diseases of the biliary tract was investigated in another study. The efficacy of the various methods, in terms of safety, morbidity, mortality, quality of life and recurrence of the disease in a follow up period of 5 years was reported.

- In a clinical study, we investigated the efficacy of repair of defects of the abdominal wall. In inguinal hernias and postoperative hernias, we used absorbable and non-absorbable materials. We investigated the anatomical structure of the defect with regard to the repair technique and the therapeutic result.

Γ) Basic research – molecular diagnostics

- An important part of research has been conducted in the field of molecular mechanisms involved in the occurrence and evolution of cancer. Specifically, we focused our research on the investigation of signaling disorders and cell cycle control disorders as well as the cellular DNA repair mechanisms in cancer. The possible mechanisms of carcinogenesis were investigated in the Barrett’s esophagus adenocarcinoma and the adenocarcinoma of the gastroesophageal junction. The transcription factor E2F-1 tended to inhibit the cellular proliferation, although there was no statistically significant correlation of the increased expression of E2F-1 with the apoptotic mechanism, whether the latter was investigated via IHC study of Caspase 3 or the TUNEL technique. Overexpression of E2F-1 was correlated with increased expression of p53 protein. The existing clinical studies on the GEJ cancer have been executed with focus on the effectiveness of the surgical method, as well as the improvement of survival and quality of life. The stages of the disease, the surgical technique, and the type of graft for the neo-esophagus (gastric tube, colon, small bowel) have also been studied. Furthermore, the value of the surgical intervention in the esophagus in patients with advanced stage esophageal cancer has been evaluated in terms of quality of life.  In a clinical study, the role of surgical therapy in the management of esophageal cancer has been investigated, with regard to the improvement of quality of life and survival. The results were compared to other anti-neoplastic therapies. Specifically, the efficacy of surgical therapy in HPB malignancies was investigated, using parameters such as the general health of the patient, morbidity, mortality and survival. The anatomical, histopathological and molecular aspects of the disease were also analyzed.

- In another study, we reported the initial positive results of gene counseling and prophylactic thyroidectomy in 19 members of 6 families with RET proto-oncogene mutations. The availability of genetic testing for mutations of the RET proto-oncogene has given us the possibility to treat the myeloid carcinoma of the thyroid gland in a timely fashion. The molecular basis of this malignancy was correlated to the histology and surgical anatomy of the disease.

- The ICAM-1 (intercellular adhesion molecule -1), a molecule that is important in inflammation and immunosurveillance, has been lately associated with carcinogenesis. The polymorphism K469E of the ICAM-1 leads to the formation of an isoform with different properties in terms of structure and molecular signaling. In an original study, we revealed the correlation between K469 polymorphism and reduced survival in patients with NSCLC, highlighting the potential role of ICAM-1 mutations in the disruption of immunosurveilance and apoptosis.

- In a laboratory study, the intraoperative use of cell salvage device was proved to be an important method for the reduction of the need for blood transfusion, especially in countries where the cost of blood transfusions is high, such as Greece. In this study, we were able to show a profit from the intraoperative use of blood salvage device with incorporated re-transfusion system.

- In a molecular study, we evaluated the impact of seropositiveness for Hepatitis B in the final response to the treatment of chronic Hepatitis C in patients with high viral load. From the results of this study that involved 185 patients, we showed that positiveness for ant-Hbc antigen did not influence the outcome of PEG IFNa – 2b in these patients. The molecular study of the virus was investigated with regard to the response to therapy and imaging findings.

- In an original study, we investigated the kinetics of the virus and the viral response to PEG – IFNa 2a – ribavirin in patients with Type 4 chronic Hepatitis C. The results of this study showed that early response to therapy was a predictive factor of final response in Caucasians but not in Egyptian origin patients. Furthermore, regardless the racial lineage, the late response to therapy was more reliable in predicting the final response than early response. The molecular type of the virus was correlated to the imaging studies of the liver.
